​Prior to surgery, you will be asked to sign the following forms (forms require Acrobat Reader):

• Advanced Beneficiary Notice 
• History and Physical
• Medication List
• Patient Rights and Responsibilities
• Notice of Privacy Practices
  

For your safety, your surgeon, anesthesiologist or primary care doctor may request certain testing be performed prior to your surgery. Once your doctor has scheduled your surgery, your list of required tests will be forwarded to the hospital. The pre-operative scheduler will call you to schedule testing. Most testing must be performed within 14 days of your surgery. Two exceptions are:     

• A History and Physical, which must be performed by your surgeon, a primary care provider or hospitalist within 30 days prior to the procedure. However, if significant changes in your medical condition occur, your History and Physical will need to be updated.
• A Type and Screen is generally acceptable within 14 days, but if you have received blood or have had a baby, abortion or miscarriage within the past three months, it must be performed within three days of the procedure.

A copy of your test results will be sent to your surgeon’s office. If there are any abnormalities that need medical attention, your surgeon’s office will contact you or your physician. You may also want to ask for a copy for your records or review your results in your patient portal.

The Christ Hospital Health Network has convenient testing locations throughout Greater Cincinnati and Northern Kentucky for pre-surgery testing. 

• Photo ID
• Insurance cards
• List of all current medications and previous surgeries
• History and Physical (if performed by your primary care physician)

• Copies of Living Will and/or Medical Durable Power of Attorney

Your surgeon may ask that an anesthesiologist evaluate you prior to your surgery if you have multiple medical problems or a history of difficulty following anesthesia from a previous operation. If so, you will be scheduled for an anesthesia consultation at the time we schedule your pre-surgery testing.

​If your physical condition changes before your surgery (e.g., you develop a cold, persistent cough or fever) or if there is a significant change to the skin where the surgery is to be performed (e.g., draining wound or swelling in the area with redness, heat, rash, tenderness and pain with pressure), notify your surgeon as soon as possible.    

We do everything we can to minimize blood loss during surgery. Your blood pressure is lowered to cut down on bleeding, and cut blood vessels are cauterized. For most routine surgeries, blood replacement is not needed. With larger, more extensive surgeries, your surgeon may ask that you consider certain options in replacing some of your blood loss or discuss an autologous blood transfusion. This is blood donated by you and later given back to you. Blood must be donated at least 72 hours, but not more than six weeks, before your surgery.

There are some medical conditions, including some heart disorders, which might prevent you from donating your own blood. If you are instructed by your surgeon to donate, your doctor will write an order for you to donate through the Hoxworth Blood Center by appointment only. Click here to view the order form.       

Call Hoxworth Blood Center at 513-558-1322, Monday through Friday (non-holidays), between 8:30 a.m. and 4:30 p.m., to schedule your donation. Donation locations include Anderson, Blue Ash, Clifton, Fort Mitchell, Tri-County or Western Hills locations. A representative from Hoxworth will let you know which location is best for you). Please have the following information available when you call. Hoxworth does charge a special service fee to collect, process, store and deliver all autologous donations.      

• Legal name as it appears on your insurance card, address and phone number
  

• Date of birth and the last four digits of your Social Security number      

• Weight, medication list and medical condition      

• Surgery date and hospital

If you are receiving anesthesia, you must have a responsible adult, 18 years or older, accompany you home from surgery. Friends, family members or loved ones can help at this time.     

Children having surgery must be accompanied by a legal guardian. Having a second adult is very helpful for the ride home. Procedures will be rescheduled if a responsible adult is not available.  
 
Taxi and bus drivers are not acceptable unless another responsible adult accompanies you.
